Toggling Repeat will make an FM instrument turn Mono if instrument is used on multiple tracks+panning.

Ran into this awhile back and I finally figured out how to reproduce it consistently.

Step 1: use one single FM instrument (can be default)
Step 2: use the same instrument on multiple FM tracks
Step 3: Do any panning stuff (ex: leave FM 1 centered, FM2: 08F0, FM3: 080F)
Step 4: Play the song
Step 5: check/uncheck the Repeat option while song is playing

Result: Tracks FM2 and FM3 will turn mono the first time you check/uncheck. You have to stop and restart playback.

Workaround: duplicate the instrument as many times as needed and use it on other tracks.

I only tested in YM2151

Oh, this happens consistently in Windows 7 on v.0.12.0
Works completely fine in Mac OS X 10.13.6 on .12
Will test Windows 10 when I'm home later

Doesn't happen in 10.

Weird. I wonder if it's sound driver related. I dual boot OS X 10.13 and Windows 7 on the same machine, so it's the same hardware. Gotta be a software driver
